Shift handover-LOTO
(1) If the handover of authorized employees is completed, the standard shift handover must be carried out.
(2) Lock the machine or equipment to the authorized replacement staff.
(3) If the authorized employee does not continue the work, take off his/her own lock.
(4) During shift change, if the replacement authorized personnel fails to hand over on time:
Authorized employees must contact their supervisors.
The supervisor locks the equipment himself.
Authorized employees off duty, remove their own locks.
When the replacement staff arrives, he/she locks the equipment with his/her own lock and the supervisor takes off his/her own lock.
Locks for personal locking procedures must be accessed by the person who locks them.
Personnel performing unconventional unlocking must familiarize themselves with the entire Lockout Tagout test procedure, contact the locker for confirmation, and obtain approval from the locker supervisor or manager before execution.
Assess whether the device is secure before unlocking it unconventionally.
