“5.11″ hydrogen sulfide poisoning accident in a petrochemical enterprise
On May 11, 2007, the diesel hydrogenation unit of the enterprise stopped maintenance, and the blind plate was installed in the rear flange of the new hydrogen pipeline. The low-pressure gas containing high concentration of hydrogen sulfide was inverted and leaked, resulting in poisoning of the construction personnel. During the rescue process, the protective measures of the rescue personnel were not implemented, resulting in an increase in the number of poisoned personnel.
The accident was caused by inadequate energy isolation, inadequate control of blind plate
operation and ineffective isolation of dangerous substances, which resulted in fire, explosion and poisoning.

On December 31, 2019, five construction workers were poisoned during maintenance operations in a desulfurization tower in an enterprise. Three of them died after rescue, resulting in a direct economic loss of about 4.02 million yuan.
Direct cause of accident:
During the maintenance of the desulfurization tower, the accident enterprise did not formulate a reasonable and reliable process disposal and isolation scheme according to the provisions, blindly discharged the desulfurization liquid breakfast liquid sealing aging, and the gas trapped in the upper space of the circulation tank broke through the liquid seal and entered the tower, resulting in poisoning of the workers.
