The accident case
At 9:30 on June 9, 2002, a petrochemical company outsourced the maintenance package unit received the task of repairing the no. 1 mixer on the east line. On duty electrician Zhou stopped east line 1 blender power supply, maintenance workers Xiao into the blender maintenance. At about 15:40, the east line bucket lifting machine block material, causing the bucket lifting mechanical and electrical source “trip”, after the shift workers clean up the block material, notify the electrician zhou on duty to restore the bucket lifting mechanical and electrical source, Zhou mistakenly east line 1 mixer power restore. At about 16:10, operator Ren mistakenly pressed the east line no. 1 blender start button, causing no. 1 blender to start, blade wound Xiao Guliang head death.

The cause of the accident
The electrician on duty seriously violated the rules and regulations, did not hang up the sign “someone is working, do not close” as required, the sense of responsibility is not strong, the wrong operation to send the wrong electricity; Operator Ren does not have the basic safety literacy, did not make clear the task of the situation, press the drive button at will, resulting in the tragedy.

Electrical equipment inspection and maintenance must be shut down and Lockout tagout
1. Preparation: Before the work, the operator or its authorized personnel will determine locking equipment, prepare locks, lock registration;
2. Inform: inform the affected operation unit;
3. Shutdown: power off the locked equipment;
4. Isolation: Perform energy isolation operations on dangerous energy sources, such as pulling down knife switch or incoming air switch;
5.Lockout tagout: Lock and tag the energy isolation device.
6. Zero energy state: fully release the remaining energy;
7. Verification: All personnel are in a safe position and try again to see if the locked device can be restarted;
8. Perform operations: repair and inspect the locked equipment;
9. Inspection and recovery: the work is finished, the site is cleaned, the remaining tools, materials and safety facilities are reset, the affected operating units are notified, the overhaul is finished, and the equipment is in a state of start-up.
