The accident after
At around 8:20 am on January 25, 2018, a labor dispatch employee of an LG production line entered the stamping machine to replace the production date mold. Instead of locking the power switch on the press, the dispatcher pressed the emergency stop button.
At this time, another maintenance personnel from other departments finished the maintenance of a part of the production line, ready to test the equipment and then handed over to work.

The cause of the accident
1. There is no communication before cross-operation, and we do not know that there are other personnel on the same equipment for maintenance work; 2.LG Company’s safety training for dispatched workers is not in place.
No Lockout tagout program, or the operator does not execute the Lockout tagout program properly!
