Reasons for ignoring LOTO
Environmental factors
Mechanical design: LOTO may be difficult or impossible on some machines/equipment, especially older equipment.
Energy isolation units are blocked or inaccessible.

Human factor
Lack of knowledge: Employees are not aware of the LOTO program.
Overconfidence: An employee’s belief that he or she can perform tasks on an energy-connected system without risk.
Pride: Employees believe they can complete tasks faster or better if they do not take the time to disarm the energy source.
Other priorities: For example, at the end of a shift, the employee believes that performing LOTO will result in lateness due to other matters after work.

Management factors
Poor execution: LOTO plan was developed but not implemented.
Lack of LOTO programs/devices: It is difficult to execute LOTO without LOTO programs or associated devices.
Mechanical design: after removing the energy source of the system, maintenance will cause loss of interest and time.
