Review the Lockout Tagout procedure
The locking procedures should be audited by the department heads to ensure that the procedures are being implemented. The Industrial Safety Officers should also conduct random checks on the procedures, including:
Are the relevant staff notified when locking?
Are all power sources turned off, eliminated and locked?
Are locking tools available and in use?
Has the employee verified that the energy has been eliminated?
When the machine is repaired and ready to operate
Are employees away from machines?
Are all tools, etc. cleared?
Are the guards back in operation?
Is it unlocked by the locking employee?
Are other employees notified that the lock has been removed before the machine is returned to operation?
Are qualified staff aware of all machines and equipment and their locking procedures and methods?
Exceptions:
This PROCEDURE MAY BE SUSPENDED WHEN THE SHUTDOWN OF AIR HOSE, WATER PIPE, OIL PIPE, ETC., WOULD AFFECT THE NORMAL OPERATION OF THE PLANT, SUBJECT TO THE WRITTEN APPROVAL OF THE DEPARTMENT MANAGER AND THE PROVISION OF APPROPRIATE AND EFFECTIVE PROTECTIVE EQUIPMENT BY THE STAFF.
When it is necessary to find out the cause of intermittent failure of the machine while the machine is in operation, this procedure may be temporarily suspended under the written approval of the department manager and with sufficient safety precautions.
