Case 1: At 9:00 a.m. on May 20, 1999, raw coal system equipment maintenance, the material in the crusher must be cleaned thoroughly before maintenance. Case 2: On November 22, 2014, a worker needed to isolate the energy of a valve near a flammable liquid pipeline in a chemical plant. He disconnected the power and closed the valve, but did not lock it because he could not find the right size lock. Seeing no one around the pipeline, he left temporarily. The meter reader saw that the pressure on the meter was 0 when he read the meter. He did not know that there were maintenance personnel repairing the pipe at this time, so he restarted the pipe. Next to the steam pipe, maintenance workers are making repairs. He covered a small section of steam pipe with insulation and opened the flammable liquid line. When the meter reader turned on the valve, flammable liquid spilled out of the pipe, fell onto the steam pipe, caught fire, and the repairman died.
