What is Lockout tagout?
This method is used to isolate and lock dangerous energy sources in order to reduce personal injury or equipment damage caused by accidental start-up of machines or accidental release of energy sources during equipment installation, cleaning, maintenance, debugging, maintenance, inspection and construction.
Why is Lockout Tagout important?
Lockout tagout may be involved in the maintenance/adjustment/inspection/cleaning of equipment, which occurs frequently and causes great personal injury, and is easy to cause crush injury, fracture, etc.
You can’t Lockout your tagout.
1. Lockout tagout is not performed (excluding identified Lockout tagout exceptions) for all activities where energy may be accidentally switched on, started or released to cause injury.
2. With the exception of Lockout tagout, alternative risk control measures are not implemented as required.
3. Lockout tagout operation instructions are not prepared, which do not cover all energy sources or are not posted on site
4. The locking personnel are not trained and authorized, or perform the locking beyond the authorized range of equipment and facilities.
5. Failed to shut down equipment, isolate and lock all energy sources as required by Lockout Tagout operation instructions, failed to use or correctly use locks and hangers, failed to control residual energy, and failed to conduct zero energy verification.
6. “One person, one lock, one key” is not strictly enforced.
7. If locks/appliances are used for other purposes, or non-standard Lockout is used for locks.
8. When Lockout tagout is executed, the affected personnel do not supervise the execution personnel.
9. When the equipment maintenance process is interrupted, the transition lock/common lock is not used, resulting in high risk exposure.
10. Contractor does not perform Lockout tagout as required by the standard.
