Usage Instruction:

1. Tuern the butterfly valve handle to the “off” or “on” position.

2. Open the lockout device and position right side under the pull handle

3. Close the lockout device perfectly to cover the valve handle.

4. Press the two halves together unit the pull handle is completely secured. Put padlocks and warning tags on the lock hole (max 3 pieces padlocks) to secure the valve handle from being unauthorized operated.
